What information do we collect?
Quantum Orthotics collects information that is required for production of your custom foot orthotics for your clinic’s patients. This will include your clinic’s name, phone number, and address as well as the names of the principal of the business, all prescribing practitioners and administration staff with a login to our system.
We will also collect the names of your patients to match with each pair of orthotics.
If you provide your email address, you can opt to be sent marketing information or you can choose to only receive communication relevant to our business dealings with you.
We will never pass on or sell your email address to a third party.
How is it collected?
This information is collected in the following ways:
• In person or by telephone or email from you to one of our representatives when setting up your clinics and prescribers on your account.
• Via the Business setup portal on which you can apply for a login directly.
Ensuring your information is correct and up to date
From time to time we may ask you to update your clinic information including which prescribers may have left or joined your practice.
Accessing and correcting your information
At any time you can request any information we have about your clinic or prescribers and we will provide it to you in reasonable time. You can also request that we close your account.
If we close your account we will still need to retain some information for tax and reporting purposes.

When a prescribing practitioner or delegate administration staff member associated with your business has submitted a script through our online portal or via email or delivered with casts or foam boxes, the clinic principal accepts the costs of making the corresponding pair of foot orthotics.
A grace period of 8 work hours is in place from the time the form is submitted.
After this grace period, the custom orthotics will be produced and the clinic associated with this account will incur the cost for said orthotics according to our price list.
A 100% satisfaction guarantee applies to all orthotics produced by Quantum Orthotics. If you are not entirely satisfied with the product we have provided, you will either receive a replacement pair or your money back.
